What you share

The form collects your name, email, city, and any phone number or message you choose to include. It also records when the request was received and your agreement to be contacted. The veterinarian referral form additionally collects the veterinarian’s contact details, appointment time, the family’s name and address, their phone number, pet details, belongings and any notes. The veterinarian must confirm the family has agreed to share these details.

How it is used

Your request is saved so Love Remains can respond, discuss your questions, and understand the care you may need. Submitting the form does not subscribe you to marketing, reserve care, provide the family’s aquamation authorization or make a payment.

Where it goes

Submitted details are saved privately and sent to Michelle’s business email so she can respond. They are not displayed on public pages. The website host, email delivery service and business email provider process these details on Love Remains’ behalf.

Keep sensitive details out

Please do not send card or banking information, identity documents, veterinary records, or other sensitive documents in this form. If care proceeds, any required authorization or payment will be handled separately.

Your choices

To update your details, withdraw a request, or ask for it to be removed, email info@loveremainspetaftercare.com. You can also email your questions directly instead of using the form.
